A captive finance desk asked for a pre-disbursement application check on a stalled batch. Across three days we flagged facilities where the approval matrix required a second signature above a threshold that staff had been rounding down. No customer was contacted; packs returned to underwriting with a clear flag list. The client later commissioned a fuller origination sampling review on the same book.
